This is an idea to generate the pattern according to your table. If I have misunderstood something, you may still be able to easily figure out the fix from this basic PWM generation concept.

You very naively believe to make your circuit work using a capacitive divider. Usually several voltage sources are used.
Don’t you notice that the amplitude of the pulses gradually decreases over time as the capacitors discharge?
You were shown a different, workable way.
